They send the application before each primary i believe. That was due to a law passed by the pa repubs. It asks for drivers license or state i.d. and last 4 digits of ss number. Also has provisions if u don't have a license. I would check to be sure that your application is legitimately from your state. Just google to see what they require. Other forms may be from companies mining for your info but just check your state website for requirements
