New patient fee at Providence (with Bluecross ins.) [View all]

My old doctor's office shut down, so I had to find a new doc.
I made an appointment with a doctor at a Providence facility for a yearly physical.

Just got the bill from our insurance: $260+ owned by us.
They charged my insurance $580+ for "Office O/p New Mod 45 Min."

Anyone familiar with these kinds of charges? I've never seen it before, and it seems awful steep if it's some kind of new patient charge.
