New patient fee at Providence (with Bluecross ins.)

My old doctor's office shut down, so I had to find a new doc.
I made an appointment with a doctor at a Providence facility for a yearly physical.

Just got the bill from our insurance: $260+ owned by us.
They charged my insurance $580+ for "Office O/p New Mod 45 Min."

Anyone familiar with these kinds of charges? I've never seen it before, and it seems awful steep if it's some kind of new patient charge.

Also is the doctor in network? This was billed as a new patient visit, moderate at 45 min. It does not appear to be billed as annual. Somewhere on the EOB(explanation of billing) there should be a code or a footnote explaining the charges.

The insurance gave me a "discount" because he was in my network. I guess otherwise I would have owed the whole $580.
Just don't understand what that is and why it's so much. I'll look into the codes. Thank you again!
