She pointed out at the time things fell apart. Joe was being overworked and his age limitations were not being considered by his staff. I think she meant she should have pointed that out, whether it was him pushing himself too hard (the man was driven after all) or his staff not building in enough time for him to get some needed rest. And that to me was what was "reckless"
