dependency and addiction, most states do not allow more than a 90-day prescription with reevaluation either required or expected, depending on how the law is written for Schedule IV controlled drugs. That said, it sounds like your practitioner is requiring a recheck every 30 days — which, while unlikely to be a requirement by state law, is likely a policy within the practice, especially if you are new to them as a patient. As you mentioned, it may also be that your insurance company is requiring more stringent limits on Ambien prescriptions.

I can imagine it is very frustrating, given that it had not been the case in the past. Maybe your new provider can advise you if there is a way to get around it. (NOT STATE OR FEDERAL LAW, OBVIOUSLY, but if this is a clinic policy only, there might be some room for a workaround)
