Hydrogen formation took place at Three Mile Island and Fukushima. In Fukushima, the explosions the world saw were caused by hydrogen.

It’s widely believed that there was a hydrogen explosion in the reactor at Three Mile Island. Hydrogen was likely involved in the explosion at Chernobyl as well.

If you watch the 1980 documentary, “The Invisible Flame” they discuss producing hydrogen using a nuclear reactor in a controlled fashion. (Start around 12:50.)
