Premiums. Consider those on Social Security, the average benefit is $1200 a month, out of this benefit the cost of Medicare is $104 a month. Some only have their Social Security to survive on so it costs. I am not complaining about Medicare, it sure has helped many people who would not be able to afford care. One of the benefits seniors will see from ACA is closing of the prescription donut hole. After $2500 in prescriptions one would have to pay the next $2500 before the plan picked up again. This is the same people who gets on avg $1200 a month. There are going to be some problems, mostly in states where a GOP governor refused to set up exchanges denying their citizens opportunities to purchase lower cost health insurance.
