They never happened in the real world, but advanced dementia patients like Trump lose their ability to discern between reality and imagination. Trump wasn't lying. This call really did happen in his mind. He probably dreamed it many times as the details started to solidify.
I recently had a dementia patient tell me that one of her dear friends had passed, but the following week she received a phone call from the deceased that showed up on caller ID as the deceased. She answered it. She heard the voice of her friend saying, "It is OK. I'm alright", and then the call ended. The family later told me they checked the phone and there had been no such call. This is how dementia works.
