Who pays the $100,000 fee and how does Trump stuff it into his pockets?

Why would people outside the US pay a $100,000 fee? You don't need a US visa if you're outside the US. Do they pay it every time the visa holder returns home and then back to the US?

You know Trump is stuffing that fee into his own pockets but the mechanics of it are unclear.
