And it's certainly broadly true that contemporary view of morality would have seen some of the flaws sufficient to not pass it into the Constitution
But within an imperfect system, there were also other arguments, accepted by people representing low slave population areas which is why the Electoral College passed congressional muster in the writing of the Constitution.
I would argue with the huge shift to urban living that accompanied mechanization and industrialization there has been a huge shift in the balance of Americans toward perspectives that mostly benefit Urban and now, Suburban residents. Moving medical care to the centers of population would be one of the costly shifts following notions of capitalist efficiency over human caring. But also urban residents have for the most part no concern for maintaining industries and ways of life that aren't part of the megalopili
