This photo in my humble dental opinion definitively shows his lower arch. It is clear that he has a splint or implant restoration that gives him only a first premolar to first premolar occlusion.
In dentistry we generally prefer to have a second premolar occlusion inasmuch as it disproportionately supports the facial height more consistently and gives the patient a greater ability to masticate or chew food.
It does explain why his face is significantly collapsed when his mouth is closed and his lower jaw is forward. He has lost facial vertical dimension. My guess is that its a reconstruction on top of his natural teeth or whats left of them. If it were an implant reconstruction it would almost certainly go back to the first molar or even beyond to the second molar. But then bc again, it is possible that the original restoration extended further posteriorly, but that he lost the back implants, so the case had to be shortened to prevent cantilevering off the anterior implants.
What it might also explain, interestingly, is his McDonalds diet addiction. Big Macs are easy to chew -believe me I have heard this from many patients over the years. This of course is in addition to the other reasons he eats them.
This is mostly conjecture but it would be consistent with an 80 year old man who has for whatever reasons, had multiple reconstructive procedures accomplished in his lifetime. His drug use could be quite contributory in this regard. I wont get into the details but trust me, it isnt out of the question.
