It gets filtered if the system is working properly, but everyone is breathing the same air for the entire time you're on the plane. The longer the flight, the more time everyone breathes the same air. You can wear a mask if you want to, but it probably won't matter much.

I suggested that my niece wear one when she heads to Europe for her wedding. She already got sick twice from flying just in the last two months.
